What Is Gynecomastia?

Gynecomastia is the enlargement of male breast gland tissue. This enlargement can be caused by an excess amount of fatty tissue, glandular tissue, or a combination of both. Gynecomastia affects men of all ages, whether they are newborns, boys going through puberty, or older men.

Males who are going through puberty often experience gynecomastia because of their changing hormones. Fortunately, this type of gynecomastia is usually resolved within six months to two years without any treatment.

Gynecomastia in adults is much more common than is generally recognized, affecting an estimated 65% percent of adult men. But for many men, it does not go away. The symptoms of gynecomastia can range from a little bit of protrusion of the nipples to severe protrusion, where the male chest almost looks like female breasts. Breasts are considered to be inappropriate on a boy’s chest. Psychologically it is a very disturbing condition as boys are supposed to have flat, muscular chests. Gynecomastia is a very embarrassing condition for men, reducing their willingness to take their shirts off at the beach or take part in sports. Luckily, gynecomastia is almost entirely correctable with surgery.

What Are Some Other Causes For The Development Of Gynecomastia?

The preliminary cause of gynecomastia is hormonal imbalance, where there is a reduction in hormone testosterone compared to the hormone estrogen. Some illnesses or medical conditions like thyroid dysfunction, liver cirrhosis, kidney issues, etc., can also cause gynecomastia.

To fix gynecomastia with gynecomastia treatment, Dr. Vasant Gholve, of Arra Aesthetics, Pune believes in identifying the cause of the problem first.

How Is Gynecomastia Classified?

Depending on the volume of the breast and the skin elasticity, gynecomastia or male breast enlargement can be classified into four different grades. Dr. Vasant Gholave, Aesthetic Plastic Surgeon and Founder of Arra Aesthetics, Pune, shows the classification as follows with the physical indicators:

  • Grade 1 Minor breast enlargement without skin excess.
  • Grade 2 Moderate breast enlargement without skin excess.
  • Grade 3 Moderate breast enlargement with minor excess of skin.
  • Grade 4 Marked breast enlargement with skin excess.

How Do I Know If I Have Gynecomastia?

Gynecomastia is often physically visible. The most common symptom is an enlargement of the breast area, wherein you have breast tissue that’s greater than 0.5 centimeters (cm) in diameter. If you happen to have any sort of breast swelling, it’s a good idea to have a doctor physically check if it’s gynecomastia or another health condition. The doctor will carefully evaluate your breast tissue, abdomen, and genitals, and have you undergo blood tests (to check hormone levels) and, possibly, mammograms.

What Is the Treatment for Gynecomastia?

The treatment for an enlarged male breast tissue will depend on its cause. If the underlying cause is obesity, your plastic surgeon will recommend a change in diet and lifestyle, since returning to a normal body weight might reduce the size of the breasts. If it’s due to drugs or medications, then the use of those needs to be stopped.

If the enlargement of the breasts cannot be reversed by diet and exercise alone, then a male breast reduction surgery is the only treatment option. Male breast reduction surgery, also known as man boob surgery, enables the removal of excess tissue from the male breasts; this can be glandular tissue, fat, and skin.

The preferred technique of gynecomastia surgery is decided by the cosmetic surgeon and is subject to the amount of tissue that needs to be removed. Overall liposuction is the gold standard mode of treatment. When a larger amount of tissue needs to be removed during man boob surgery, the surgeon may choose a tissue excision procedure just like breast reduction surgery for females. The decision regarding the correct type of technique is carefully taken by your plastic surgeon so as to minimize the chance of a “re-do” surgery in the future.

The effects of man boob surgery are usually permanent as long as the patient is willing to maintain a healthy lifestyle. If there are serious fluctuations in body weight, the problem may return over time.

How Is Gynecomastia Surgery Performed?

At Arra Aesthetics, Pune, the techniques used for Gynecomastia surgery are Liposuction and Excision. Dr. Vasant Gholave will decide the surgical technique depending on patient preference and the nature of their problem.

Gynecomastia Surgery By Liposuction:

A patient can be put to sleep, given IV sedation, or be given just local tumescence anesthesia or general anesthesia. One or two ¼ inch incisions are made on each side of the chest. A long-acting anesthetic along with fluid is placed inside the breast tissue. After the surgeon injects a long-acting anesthetic and fluid into the breast tissue, he waits for it to distribute before placing small tubes through incisions in order to remove fat from those areas. Those incisions are closed with just several small sutures that may be dissolvable or are removed after about 10 days.

Gynecomastia Surgery By Excision Technique:

Excision means cutting out tissue. This can be done with IV sedation, general anesthesia, or local anesthesia. An incision is made from about 5-8 o’clock on the areola and then the excess tissue is removed. Self-dissolving stitches are placed. Many times excision is combined with liposuction.

Dr. Vasant Gholave will use drains only when he has to take out very large amounts of breast tissue. He will tell the patient ahead of time if this will be necessary.

Most male breast reduction surgery is completed in as little as an hour in case of liposuction and up to 2-3 hours in case of excision.

What Happens During Recovery After Gynecomastia Surgery?

The final result of gynecomastia surgery will be visible almost instantly; however, there will be swelling and bruising that will make the skin look puffy.
A dressing is put over the wound to assist with healing. A pressure or compression garment needs to be worn for 6 weeks after the surgery. Wearing a compression garment is important as it helps to minimize swelling and speed up the healing process by supporting your newly contoured chest. Gynecomastia is a daycare surgery. It will take about a week to be able to resume your work. If you require more time, Dr. Vasant Gholave can provide a certificate for work if required. You can ease back into exercise after 4 to 6 weeks.

Generally, gynecomastia surgery is relatively pain-free. Most patients report discomfort after male breast reduction surgery, but no significant pain.

Dr. Vasant Gholave will give you a prescription for pain relief medication following surgery.

What Are The Risks Of Gynecomastia Surgery?

Surgical correction of gynecomastia is a popular procedure that provides consistent results. It should only be performed by a specialist plastic surgeon to minimize complications.

Like any other surgical procedure, gynecomastia surgery may cause some unavoidable complications such as infection, bruising, delayed healing, pain, and loss of sensation, but these are usually self-limited and if need be, easily treated. In general, for male breast reduction, other risks may include visible scars, change in nipple sensation, irregular chest contour, and excess skin.

Dr. Vasant Gholave will ensure that scarring from your breast reduction is minimal. Incisions are positioned to be least noticeable along the lower border of the areola and inframammary fold. Most gynecomastia surgery scars have usually faded within 6-12 months after surgery. To hasten the scar remodeling Dr. Vasant Gholave uses the latest RecoSMA Laser treatment for Scar Rejuvenation.

With any breast surgery, there is a chance of a change in nipple sensation. It is variable from patient to patient. Some patients feel changes in sensation, while others do not experience any change at all.

Are The Results Of Gynecomastia Surgery Permanent?

The good news is that most individuals who suffer from gynecomastia can get rid of it permanently with surgery. However, certain factors might cause gynecomastia to return after surgery.
Gynecomastia patients are informed about the possibility of their gynecomastia coming back. The effects of gynecomastia surgery or male breast reduction surgery are long-lasting, however, if you gain weight following the procedure, or if you start medications again which caused the gynecomastia in the first place, fat cells can return to the chest area. Few cases in which liposuction was done and the skin does not retract properly might require skin excision later. At Arra Aesthetics, Pune, our plastic surgeons do a careful preoperative assessment and have a frank discussion with the patient to ensure that the patient has realistic expectations.

Frequently Asked Questions About Gynecomastia

What is the difference between gynecomastia and chest fat?

Gynecomastia or male breast enlargement is a condition that causes men to develop enlarged breasts. It can occur due to an imbalance in hormone levels, certain medications, or obesity. In most cases, gynecomastia surgery may be necessary to correct the problem.
Chest fat or pseudogynecomastia is simply excess fat that has accumulated in the chest area. It is not caused by an imbalance in hormone levels and usually does not require surgery to correct it. Generally, chest fat or pseudogynecomastia can be reduced through a healthy diet and exercise regimen. In the case of gynecomastia, a hard lump may be felt beneath the nipple or areola. However, no firmness or hard mass is felt under the nipple or areola in case of chest fat or pseudogynecomastia.

Can gynecomastia be prevented?

Generally, gynecomastia can’t be prevented, since the primary causes of gynecomastia are hormonal imbalance and certain medical conditions. Fortunately, gynecomastia can be effectively treated with gynecomastia surgery.

How does gynecomastia affect my body?

Gynecomastia can have a number of physical effects on your body. In addition to causing the breasts to become enlarged, it can also lead to nipple discharge, tenderness, and pain. The condition can also cause psychological distress, leading to feelings of self-consciousness, embarrassment, and anxiety.

How do I know if I need gynecomastia surgery?

There are a few key things to look for when considering whether or not you may need gynecomastia surgery. First, look at the size and shape of your chest. If you have large, protruding breasts that are out of proportion with the rest of your body, it’s likely that you have gynecomastia. Additionally, if you experience discomfort or embarrassment due to the size or appearance of your chest, gynecomastia surgery may be right for you.
If you’re unsure whether you have true gynecomastia or simply excess fat in your chest area, it’s important to consult with a board-certified plastic surgeon. They will be able to assess your individual case and recommend the best course of treatment.

Is gynecomastia surgery painful?

During gynecomastia surgery, the surgeon will use general or local anesthesia to numb the area so the patient will not feel any pain or discomfort.
A minimal amount of pain may occur during the recovery period, but Dr. Vasant Gholave will prescribe pain medication to help ensure your comfort. Additionally, he will provide you with detailed post-operative instructions to help facilitate a smooth and speedy recovery.
