Anatomy of hair growth-anagen/catagen:
Scalp में हेयर growth की 3 phases होती है. Anagen यानी growth phase, catagen यानी degradation phase और telogen यानी resting phase. हमारे सर पर normally एक से डेढ़ लाख हेयर फॉलिकल्स होते हैं. इसमें से 85% हेयर फॉलिकल हेयर ग्रोथ phase में होते हैं. जो hairs हमारे सर पर दिखते हैं वह anagen phase के होते हैं. यह cycle 2 से 6 साल का होता है.

What tests needed for hair loss assessment?
Hair loss assessment में nutritional deficiency या hormonal disorder को पहचानना जरूरी होता है क्योंकि अगर हम basic cause ऑफ hair loss treat नहीं करेंगे तो कितनी भी हेयर लॉस की advanced ट्रीटमेंट करते है तो भी रिजल्ट अच्छे नहीं मिलेंगे. Commonly CBC,ESR,Sr.Ferritin levels,Vit D3 levels and Vit B12 levels कि मैं जांच करवा लेता हूं. अगर जरूरत पड़ी तो Zinc और Male और फीमेल हार्मोन levels (testosterone,DHEA) females में करवाता हूं. कई चुनिंदा पेशेंट में जिनमें inflammatory disorder suspect करता हूं उसमें scalp skin biopsy करना चाहिए ताकि definitive डायग्नोसिस हमें मिले. तो चलिए हेयर ग्रोथ की अलग अलग मेथड की तरफ बढ़ते हैं
What are different hair growth methods?
अगर आप male पेशेंट हो और आपका थोड़ा सा ही hair thinning है तो bald scalp को ढकने के लिए आपके हेयर को 10 cm तक बढाइए, हेयर स्टाइल चेंज करें और parting लाइन जहां से hairs को दाएं और बाएं मोड़ते हैं वह लाइन को चेंज करें. इससे काफी हद तक आपको benefit हो सकता है.
- Lifestyle modification: stress एक well known risk factor है हेयर लॉस के लिए. Stress कम करने के लिए आप meditation,योगा कर सकते हैं. Eat heathy food. जिसमें मल्टीविटामिन और फल और हरी पत्तेदार सब्जियों सहित एंटीऑक्सीडेंट से भरपूर भोजन का सेवन करना चाहिए. हर दिन आपको कम से कम light exercise तो करनी ही चाहिए.
- Medical line ऑफ ट्रीटमेंट: मुख्यता इनका इस्तेमाल further हेयर लॉस कम करने के लिए होता है. कई बार यह hair regrowth भी करते हैं.मेडिकल ट्रीटमेंट में मुख्यता minoxidil और finasteride यह FDA approved treatment है.Minoxidil 2% या 5% की strength में इस्तेमाल होता है. Scalp पर लगाने के बाद यह ब्लड सप्लाई बढ़ाते हैं और additional हेयर लॉस prevent करते हैं. Finasteride एंटीएंड्रोजन ड्रग है जो testosterone का dihydrotestosterone में conversion रोकता है. 1 mg daily डोस लेने के बाद इसका काफी असर होता है.Off label use में Dutasteride भी यूज़ किया जाता है. हेयर specific मल्टीविटामिन जरूरी ट्रीटमेंट है.
- PRP-Platelet rich plasma therapy: इसमें 10ml ब्लड draw करने के बाद उसे centrifuge मशीन में spin किया जाता है.उसके बाद प्लेटलेट रिच प्लाजमा को separate करते है.PRP को काफी छोटी needle से scalp में इंजेक्ट किया जाता है. इंजेक्शन either इन्सुलिन सिरिंज से दिए जाते हैं या microneedling मशीन के सहारे.PRP में जो ग्रोथ फैक्टर होते हैं वह हेयर फॉलिकल के ग्रोथ में सपोर्ट करते हैं. Usually इसे में hair mesotherapy या recosma laser के साथ कंबाइन करता हूं. Optimum treatment sessions 4 से 6 होने चाहिए.
